1. Download the Word Document questionannire for speakers: spkques.doc

2. Print out and complete the questionnaire

3. Mail the questionnaire along with requested promotional materials to the attention of "New Speaker Submission"

4. Please allow a minimum of two to three weeks before calling Valerie Morris at 865-546-0000

5. Sports Stars USA does not return any promotional materials

6. It is understood and agreed to that by sending Sports Stars USA promotional material you are giving Sports Stars USA expressed permission to publicize and promote artist/speaker in any manner it deems necessary, including but not limited to brochures, direct mail pieces and placement on any website controlled by Sports Stars USA. By sending promotional material I give permission for Sports Stars USA to edit the material in any manner they deem necessary. In submitting copyrighted material to Sports Stars USA to place on it's website, artist/speaker warrants that all necessary permissions have been obtained, and agrees to hold harmless and indemnify Sports Stars USA for all costs, expenses, attorney's fees and judgements resulting from any claim of copyright infringement. Sports Stars USA looks forward to a long term prosperous working relationship.
